Person with Special Needs Emergency Info Form

This voluntary form is used  to provide detailed emergency information on a family member with special needs who may require an emergency response. It will be used by the Montgomery Township Police Department and the Somerset County Communications Center to flag homes for the 911 data system and our records management system in an effort to provide emergency services.
